Maternity Pre-Registration

Please fill out our secure online maternity pre-registration form prior to arriving at the hospital for the delivery of your baby. We recommend that you pre-register two months in advance of your due date. This helps us to collect important information and makes your delivery day go more smoothly.

Before the delivery, please check with your insurance company or your employer's benefits office to determine the process for adding your new baby to your insurance coverage. Please note that you may be responsible for a copay or deductible, prior to delivery or on the day of delivery. If you do not have insurance, please contact our Financial Counseling office at 301-754-7195 for Holy Cross Hospital or 301-557-6195 for Holy Cross Germantown Hospital.

Even though you have pre-registered, please be sure to bring your insurance cards and photo ID with you to the hospital.

All fields marked with an asterisk (*) are required. If a required field does not apply to you, enter "N/A" or "not applicable." If you have all of your information on hand, you should be able to complete this form within approximately 15 minutes. Please note that we cannot accept incomplete forms.
