(Silver Spring, Maryland) – Holy Cross Health in Silver Spring, Maryland is pleased to announce the opening of a new Health Center in Germantown, Maryland. This brings to a total of four the number of health centers in Montgomery County owned by Holy Cross Health. With the opening of the first health center in Silver Spring in 2004, Holy Cross Health continues its commitment to improving the health of the communities it serves by providing better access to care and affordable services. Holy Cross Health is the only health system in Montgomery County that owns and operates safety net health centers.

“I am delighted by the opening of the new Holy Cross Health Center in Germantown, near the recently opened Holy Cross Germantown Hospital,” said Kevin J. Sexton, President & CEO of Holy Cross Health. “Holy Cross is dedicated to improved access and care coordination for all and this health center, our fourth, will bring these outstanding services to additional people in need in the Germantown community."

This latest addition to Holy Cross Health is located at 12800 Middlebrook Road, Suite 206 in Germantown, Maryland and will offer primary care, preventive care, screenings, chronic disease management, behavioral health, health education, and follow-up care for emergency room and hospital visits. This new location will also expand services to include pediatrics.

"In 2014 our three health centers provided more than 30,000 patient visits, " said Annice Cody, President, Holy Cross Health Network. "Opening a new health center in Germantown will improve access for children and adults who face financial barriers to primary care."

In addition to meeting each patient’s health care needs, Holy Cross Health brings a population-based perspective to patient care. By gathering key clinical indicators for patients with chronic conditions and tracking emergency department use and hospital admissions and re-admissions, the Network can target programmatic improvements to better manage patient care. This results in improved outcomes for patients with chronic conditions.
