Holy Cross Health Care Transformation Organization

Primary care practices are increasingly challenged to better manage patient care while responding to new reporting requirements and financial pressures. Health systems face the same challenges. 

In response, Holy Cross Health is forming a Care Transformation Organization (CTO) to help primary care practices successfully participate in the Maryland Primary Care Program (MDPCP).  This program provides practices with access to funding to enhance care management and reward quality for Medicare beneficiaries.

The Holy Cross Health CTO will partner with physician practices throughout the community to coordinate people-centered care — allowing you to connect more with your patients, offer a higher level of coordinated care and support, and help you meet provider and practice reporting requirements.
