Since opening in fiscal 2004, the Holy Cross Health Centers have provided more than 200,000 patient visits. These centers are the linchpin in our community benefit program, helping to create a continuum of care for area residents in greatest need.

Continuity of Care 

The centers provide a wide range of services to low-income and uninsured individuals and families to help maintain their optimal health and well-being:

  • Primary care
  • Preventive care, including health screenings
  • Chronic disease management
  • Behavioral health
  • Health education
  • Follow-up care for emergency room and hospital visits
  • Pediatric health care (located at the Holy Cross Health Center in Germantown)
  • Referral to specialty care when needed

Through these patient-centered medical homes, nurse care managers work with patients one-on-one to develop care plans and create practical strategies to help them cope with chronic illnesses—and prevent new health problems. When patients need specialty care, center staff will refer patients to physicians in our growing network of providers.

Streamlined communications between Holy Cross Health specialists and primary care providers, including a system-wide integrated electronic medical records system, ensure care coordination.

Community Benefit Program Hub 

The centers are also home base for several other important community benefit programs, including LINCS, which connects patients and area residents to a variety of community social services.

They are also home to our cadre of community health workers, including bilingual staff, who are important to our MAPS program for breast health education to women at high risk.

The centers are critical to the success of Holy Cross Health patient navigation programs, including the Transitional Care Program, which often refers patients to the centers for follow-up—and ongoing—care following hospitalization.
