Holy Cross Health Receives Grant from Maryland Health Services Cost Review Commission
Funding Supports New Regional Partnership to Reduce Avoidable Hospital Utilization
Holy Cross Health has received a $300,000 grant from the Maryland Health Services Cost Review Commission to develop NexusMontgomery, a regional partnership to reduce avoidable hospital utilization and promote wellness for seniors.
The partnership includes Holy Cross Health, Suburban Hospital, the Primary Care Coalition of Montgomery County, the Montgomery County Department of Health and Human Services along with 23 senior living communities, physicians and other providers.
The goal of NexusMontgomery is to work in senior living communities to identify individuals most at risk for avoidable hospitalization and develop a care management model that can help keep them safely at home.
The partnership will be working with outside firms that can bring technical expertise in predictive modeling and model design to ensure that a self-sustaining model is created.
This work advances Holy Cross Health's efforts to improve individual and community health through innovation, alignment and partnership, as well as the health system's strong foundation in care for seniors, both in our hospitals and in the community. The NexusMontgomery will be implemented next year.
