Creating a Strategy for the Community’s Health
The Montgomery County Hospital Collaborative implementation strategy was developed in partnership with the community and will focus on specific populations and geographic areas within our community benefit service area most impacted by the needs being addressed. Racial equity principles were used throughout the development of this plan and will continue to be used during the implementation. In addition to health promotion and disease prevention, the strategies implemented will also focus on policy, systems, and environmental change, as these systems changes are needed to dismantle racism and promote health and well-being for all members of the communities we serve.
Three themes emerged from the 2022-2025 strategic planning process. These are helping Holy Cross Health frame its community benefit initiatives:
- Attain healthy, thriving lives and well-being free of preventable disease, disability, injury, and premature death.
- Promote healthy development, healthy behaviors, and well-being across all life stages.
- Create social, physical, and economic environments that promote attaining the full potential for health and well-being for all.
Holy Cross Health's board of directors approves the annual plan of the implementation strategy and evaluates progress to ensure that goals are met. The plan becomes the multi-year guide to improving the health status of our community and targets the gaps in health needs that have been identified.
