Senior Care at Home

Given the option, many older adults would prefer to remain independent, living in their own homes, even if they have serious health problems. We understand that staying in a familiar and secure environment can greatly improve the quality of your life.

To help make it possible for you or your loved one to remain at home, we offer senior in-home care programs through Holy Cross Home Care and Hospice, a member of Trinity Health at Home. 

Our comprehensive agency for home care is Maryland-state licensed, Medicare-certified, and CHAP-accredited, which means that our care meets or exceeds the highest standards for quality.

• Our Home Care offers a wide range of senior health services, including basic and advanced home care. These include geriatric nursing care, our medical house calls program, and emotional and social support for seniors with chronic illnesses, as well as for those recovering from an acute health problem or from surgery. 
• Our House Calls program allows older adults, who are unable to get to their physician's office, to receive primary medical care at home, delivered by a nurse practitioner. Nurse practitioners are specially trained and certified to provide comprehensive medical care, including examining and diagnosing patients, prescribing medications, and arranging for lab and other diagnostic services. 
• Our Home Hospice Care program provides you and your family support to deal with the physical, emotional, and spiritual issues you may face during the course of a terminal illness.
